Patient admission
Bring complete information about the patient’s health, medicines, previous hospital stays, identity, address, contact details, employer, and insurance coverage.
Helpful items to bring
- Valid identification and complete contact details.
- Doctor’s referral, requests, test results, and a list of current medicines.
- Active HMO card, PhilHealth documents, or other insurance information.
- A copy of your advance directive, if you have one and want it applied during admission.

- Visiting hours: 10:00 AM–12:00 noon and 3:00 PM–9:00 PM.
- Children seven years old and below are not allowed to visit confined relatives or friends.

During your stay
Hospital policies
- The hospital maintains a no-smoking policy.
- Firearms and alcoholic or intoxicating beverages are not allowed inside the hospital.
- Watchers should not use vacant beds; occupied beds may be charged to the patient’s bill.
Ask the Information Desk or nurse station for current guidance for your ward.

HMO processing
Outpatient
- Present your active HMO card and valid ID to the hospital-authorized representative.
- Hospital staff verify eligibility with the HMO.
- Proceed to an accredited doctor’s clinic once verified. Emergency cases may proceed to the Emergency Room for immediate treatment.
- If a diagnostic procedure is ordered, staff request HMO approval and issue an approval slip when approved.
Inpatient and discharge
The hospital reports admission details and bill updates to the HMO. At discharge, the statement of account and detailed bill are submitted so the HMO can issue a Letter of Authority. Approved HMO coverage is then forwarded to Billing for deduction.

Benefits
PhilHealth processing

- Obtain the PHIC form and list of required documents from the billing or PHIC desk.
- Submit completed forms for review. Missing requirements must be completed before processing continues.
- Once requirements are cleared, receive an accomplishment slip.
- The approved PhilHealth benefit is applied to the hospital bill.
- Pay any remaining balance at the Cashier. Medical records and physician sign-off are finalized for claim processing.
Payment
Billing and discharge
Accepted payment methods: cash, manager’s or cashier’s check, credit or debit card for payments of at least ₱10,000, and bank transfer.
Inpatient discharge flow
- The nurse initiates discharge and hospital departments clear pending procedures, supplies, and charges.
- Billing prepares the final bill and applies approved HMO, PhilHealth, Senior Citizen, PWD, or other eligible deductions.
- The patient or authorized representative pays the remaining balance at the Cashier.
- The Cashier issues a discharge slip. Present it to the nurse station and security before leaving.
Documentation
Medical-record requests
Certified copies may include a discharge summary or medical abstract, laboratory and diagnostic results, and an operation report. Availability can vary; patients are encouraged to request a discharge summary or medical abstract before discharge to avoid delay.
Phone, text, and email requests are not accepted because the hospital must verify the requester’s identity. An authorized representative needs a patient-signed authorization letter, a copy of the patient’s government ID showing the specimen signature, and the representative’s valid ID.
